HOME ENERGY ASSISTANCE
 
Program Dates: November 30 through March 30
 
Grant Amount: $125 for home energy costs
(per household, per year)
 
Open to current dialysis patients
 
Applicants must complete the online information form below to receive an application. The application will be sent to you via U.S. Mail or emailed within 24 hours. Please complete all forms and return with a photocopy of your Past Due home energy bill in its entirety and photo identification. The name of the applicant must match the name on the energy bill.
 
Approved applicants will be notified by U.S. Mail and/or email. All payments are mailed directly to the home energy provider within 5 days of final approval.
